Legal CT Resident at least 18 years of age
Must have valid proof of CT residence (ie. Driver’s License or state ID card)
Must have a qualifying medical condition as listed by the Connecticut Medical Marijuana Program (MMP)
Cost of Medical Marijuana Evaluation
First Time Evaluation $140
Renewal $110
Low Income or Veteran (Must Show Proof) $100
Pre-Visit Requirements
Must have medical records documenting the qualifying medical condition(s) that makes you eligible for the medical card. You can either bring your medical records with you to the visit OR you can upload it to the patient portal prior to the visit.
How long does the visit take?
Visit takes approximately 30 minutes
